Melasma is also known as chloasma. It is common skin problem in women. It appears as gray-brown patches on the face i.e. cheeks, bridge of their nose, forehead, chin, and above their upper lip. It also can appear on other parts of the body following intense sun burn such as the forearms and neck. Melasma is more common in people with naturally brown black skin type.

The cauMalesmase of melasma is complex. However, the pigmentation is appearing on the face due to overproduction of melanin by the pigment cells, melanocytes, which is taken up by the keratinocytes (epidermal melanosis) and/or deposited in the dermis (dermal melanosis, melanophages). Known triggers for melasma are written below.

  • Prolonged sun exposure cause expose to ultraviolet radiation (UVR) deepens the pigmentation because it activates the melanocytes to produce more melanin. This is one of the most important avoidable risk factor.
  • Pregnancy triggers melanine production due to hormone changes in women. The pigment may or may not fade post-delivery.
  • Hormone treatments such as oral contraceptive pills hormone replacement, intrauterine devices and implants may contain oestrogen and/or progesterone that triggers melanine production.
  • Certain medications including new targeted therapies for cancer.
  • Chemical sun screen, scented or deodorant soaps, toiletries and cosmetics may cause a phototoxic reaction that triggers melisma and it may persist long term.
  • Hypothyroidism (low levels of circulating thyroid hormone)

The effect is the skin tone improvement and removes pigmentation on epidermal or mixed melasma. The Q-switched laser is FDA approved for melasma and pigmentation treatment.
